There are five components to our registration process:

• Family Registration Form
• Checklist & Payment (with team gear)
• Code of Conduct
• Liability Waiver
• USA Swimming Association Membership

All five must be complete to become a Swim Dog, and they are discussed below.

 :: 2008 Swim Dogs Dues

$10.00 discount for additional family members.  Fees are due by the 5th of the month.  There are no attendance requirements.  Schedule and fee adjustments will be considered on a case-by-case basis.

* At the time of registration, an annual membership fee of $100 is required to cover USA Swimming annual dues, team t-shirt, and cap.

In 2008, Swim Dogs implemented a Code of Conduct for its athletes and families.  Please download, read, and complete this form for each swimmer from your family:  Code of Conduct.pdf.

 :: Liability Waiver

In 2008, Swim Dogs implemented a Liability Waiver for its athletes and families.  Please download, read, and complete this form for your family:  Liability Waiver.

 :: Annual Association Dues

In addition to our monthly fees, every Swim Dog is required to register for USA Swimming:  There are no exceptions!

Please download and complete the USA Swimming Athlete Registration Form:  2009 USA Swimming Athlete Registration Form.pdf.  Bring this form along with your annual dues ($55) to complete your Swim Dogs registration.

You can combine your Swim Dogs fees and USA Swimming Fees into one check made payable to Swim Dogs.

For our Masters swimmers, we would also like you to join the United States Masters Swimming association (USMS).  Please download and complete the USMS form:  USMS (Masters) 2008 Individual.pdf.

Starting in 2008, every swimmer is required to sign up for USA Swimming.  As a benefit to our masters swimmers, Swim Dogs will cover the additional USMS annual dues.
